Scottish Welfare Fund - First Tier Review
Local Councils are responsible for the approval of Scottish Welfare Fund applications. If your application has been refused in the last 28 days you can request a review by completing this form. This review is called a “First Tier Review” – we sent you a leaflet explaining the process with your letter.

It’s helpful to have your refusal letter to hand when completing the form – using the reference number that appears on the letter makes filling out this form much quicker.
If you don’t have your refusal letter to hand, or if you’re applying for a review on someone else’s behalf and don’t have a copy of their letter, you’ll need either a few other details instead:
- The applicant’s date of birth
- The applicant’s national insurance number
- The applicant’s address with post code
- The applicant’s telephone number
- The applicant’s email address
If you’re applying on someone else’s behalf, you’ll also need a signed consent letter from the original applicant to authorise the request for a review. We won’t review their application without one.
